Basic Steps in the Name Change Process

    1. Fill out the Name Change Packet. Get forms here.
    2. File forms at Court House. Fee is $276.00
    3. Pick a court date allowing time for step 4.
    4. Publish in local paper for 3 weeks.
    5. Attend court. Bring ALL documentation. Judge rules on request.
    6. Update name information at various agencies.
